What you'll do:
Develop relationships and collaborate with primary care tea to conduct social services assessments to determine the appropriate needs for each member.
Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
Navigate managed care plans for community services and programs.
Requirements:
Experience working with patients in need of social services.
Experience with assessments and planning for social services.
Working knowledge of different diagnosis of common health conditions.
Education and Experience:
Fully licensed LMSW in NE.
Master's degree in Social Work.
1+ years' experience in clinical social work
Fluency in Spanish or other language spoken by people in the communities we service.
