Adecco Healthcare & Life Sciences is hiring a Case Manager RN in Saint Cloud, Florida! This role is in person / on site. Please read below and apply with an updated resume for consideration:
Type: Four-month contract, opportunity for extension
Schedule: Monday through Friday, 8am to 5pm
Pay: $31.50 to $33.55 an hour
Responsibilities of the Case Manager RN:
· Manages transition of care patients, triages, scheduling, hospital tracking and follow up
· Coordinating patient needs with hospice/vitas
· Responsible for managing hospital updates, discharges, and scheduling accordingly.
Qualifications for the Case Manager RN:
· BSN degree, preferred
· Active, unrestricted RN licensure in Florida
· Prior case management experience is a plus
· Bilingual with English and Spanish is a plus
