Program Overview
TheRoads Home program is designed to serve very low-income, primarily homeless Veterans and Veteran families. The program assists Veterans in exiting homelessness to permanent housing and in maintaining a permanent housing placement. Services provided by Insight Housing include street and venue-based outreach, case management, permanent housing placement, employment services and assistance in obtaining VA and other benefits.
**Position Summary
TheSSVF Housing Case Manager is responsible for the provision of case management and housing stabilization services to homeless veteran households assisted through the Supportive Service for Veterans Families (SSVF) Program. The Case Manager provides a variety of services to address housing barriers and to increase housing stability, including service coordination to ensure that participants are connected to the VA and other necessary supportive services. This position is based in Amador County and will require weekly trips to the Sacramento office.
Essential Duties and Responsibilities
Conduct and document initial screening and eligibility determination for all new client referrals
Provide street and venue based outreach to homeless veterans
Participate in the Veteran By Name List efforts
Provide intensive housing and vocational case management to veterans and their families
Conduct home visits
Actively involve clients in the design and delivery of supportive services by ensuring they have an active voice in their goal/service plans
Collaborate with the Housing Specialist to maintain positive landlord relationships
Develop individual budgets and service plans with each veteran client
Provide service coordination and linkage including: Veteran Services Enrollment, crisis intervention, vocational and employment assistance, general health and dental services, income support and benefits, substance abuse treatments, mental health services and consumer and family involvement
Conduct weekly meetings with participants to identify short and long-term goals, money management, and employment education issues
Assist participants in retaining housing and maximize their independence and self-sufficiency by providing linkage and referral to appropriate community services and resources
Maintain and secure comprehensive case files and prepare and submit all programmatic reports as required
Provide information and recommendations to the Program Manager regarding program evaluation and modification to better meet resident needs, community needs, funding requirements and Insight Housing's mission
Co-facilitate housing clinics, groups and other Housing Case Management Team (HCMT) activities
Oversee and coordinate SSVF client intake process and initialassessment, create and maintain new client files electronically
Maintain HMIS database records, including client intakes, program entry and program exit
Ensure that all data is entered into HMIS within 24 hours of service delivery
Participate in department staff meetings and case conferences
Build relationships with landlords in Amador County or surrounding areas,
Create and maintain a landlord listing database
Match landlord qualifications with client needs
Assist with completing the VA consumer survey for all clients.
Perform or arrange housing inspections prior to move-in
Assist in negotiation of rents with landlords
Collect and keep files of income certifications, calculations of subsidies and lease approvals for participants direct lease contracts.
Generate and maintain individual housing plans with program clients
Conduct regular home visits to clients who are housed as part of the Veterans After Care Program (VAC)
Ensure that all regulatory agreements, contracts, and fair housing laws are met as they relate to housing
Provide program participants with the housing resources needed to reach their permanent housing goals
Provide individual and in group education regarding housing retention, tenancy skills and money management
Coordinate eviction prevention services and follow up with landlords
Adhere to laws regarding confidentiality and reporting requirements; maintain knowledge of current HIPAA certification standards
Assist with the client intake process and initial assessment; create and maintain new client files electronically as needed
Document all services provided either on paper or through HMIS data base, including client intakes, program entry and program exit
Participate in weekly department staff meetings and case conferences
Perform other related duties as required by supervisor
Qualifications, Skills and Abilities
BA/BS in Social Work or 3 years of experience required.
Minimum of two years previous case management experience
Experience working with veterans required
Knowledge of homelessness, mental health and substance use required
Excellent knowledge of local housing market
Capacity to set priorities in consultation with supervisor
Good conflict resolution
Ability to meet deadlines and complete multiple tasks in a timely manner
Excellent written and oral communication skills
Ability to maintain professional conduct, attitude and appearance at all times
Capacity to work independently and as part of a team
Must have experience with Microsoft Office, databases, and be comfortable learning and working with different computer programs
Special Requirements
This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies.
Must be able to receive and maintain criminal and credit records clearance.
Physical Requirements
