JOB SUMMARY AND PURPOSE
Under the direct supervision of the Program Manager, and working in collaboration of the Medical Director, Clinical Director and MAT AOD Specialist, the Licensed Vocational Nurse (LVN) provides professional nursing services for this residential substance use program. As a patient advocate, the LVN will also provide limited physical examinations, medication support, detox withdrawal management care, and MAT educational services
Clients are assessed using the full American Society of Addiction Medicine (ASAM) assessment and receive the following services: Education and counseling groups, individual counseling sessions with Substance Abuse Specialist and/or a therapist, case management, transportation, and random drug screening.
DUTIES AND RESPONSIBILITIES
Provides nursing services including: monitoring of withdrawal symptoms, monitoring of medications including MAT, monitoring vitals, direct communication with treatment medical providers on or off-site for referrals to other provides and community health resources, health instruction to patients, and instruction and in-servicing to counseling line staff including monitoring of self-administration of prescribed medication.
Designs and provides a comprehensive health services plan for each client that includes a broad range of medical services including assisting clients with physical and emotional needs, and referral to appropriate service providers when needed.
Documents patient's current medication list, including: name of prescriber, names of medication, dosages, frequency, duration and start date
Participates in weekly case review with Program Staff to ensure best quality care for clients, and continuous improvement of services.
Acts as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients
Maintain electronic health records and hard copy documentation of all services provided
Other Tasks that may be required by the Program Manager
Qualifications
REQUIREMENTS:
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
EDUCATION:
Current and valid California Licensed Vocational Nurse
Valid Cardio Pulmonary Resuscitation (CPR) certification
Must have completed ASAM I and ASAM II for access to EHR
EXPERIENCE:
One year of work experience as an LVN
Work experience serving substance abuse clients
PREFERRED QUALIFICATIONS:
Culturally sensitive and demonstrated ability and effectiveness working with mental health and substance use disorder
Experience working with the homeless population
Bilingual (English/Spanish - written and oral)
