Description
POSITION SUMMARY:
Person in this position will be responsible for Navigation in the Adult Reentry Grant Warm Hand Off program. The Navigator will utilize a Housing First approach to the delivery of its Warm Hand Off Reentry services to formerly incarcerated individuals who are experiencing or at imminent risk of homelessness in an effort to break the cycle of homelessness and incarceration. The services provided will assist in reintroducing formerly incarcerated individuals, including serious and violent offenders, back to their respective communities, family systems and other positive supports. Essential Job Functions include those listed below.
ESSENTIAL JOB FUNCTIONS:
Case management a caseload of participants, up to 20-30 at a time.
Have a strong working knowledge of housing services, supportive services, and community resources.
Ability to link clients to needed programs and provide warm handoffs; including increased access to effective community based mental health, behavioral health (including the option to use 12-Step program), and primary care treatment.
Heavy documentation; CDS data entry
Assist clients in securing documentation and services needed to obtain housing.
Provide wraparound services for individuals and their families to ensure their successful transition to independent living and successful community reintegration by arranging, coordinating, monitoring, and delivering services related to meeting participants' housing needs and helping them to obtain stability.
Link participants to programs for literacy, vocational training, education (GED, community college), and job readiness skills (interpersonal and interview skills, resume preparation, job search assistance).
Assist clients in applying for housing vacancies, completion of supportive and subsidized housing paperwork, and monitoring progress to completion.
Ensure clients transition smoothly to permanent housing.
Follow clients through and among services to achieve housing stability and retention.
Navigate clients until they are linked to appropriate housing intervention or services are no longer needed.
When applicable, attend required Fresno Madera Continuum of Care (FMCoC) CES and Navigator trainings.
For participants experiencing homelessness, provide weekly updates to Master by Name List and participate in weekly FMCoC case conferencing meetings.
Provide all services in linguistically and culturally appropriate manner, including use of interpreters for those with limited English proficiency.
Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
Other relevant duties as assigned.
Qualifications
ESSENTIAL QUALIFICATIONS:

Must be culturally/linguistically sensitive to populations served.
Must be able to successfully obtain and maintain a criminal background clearance.
Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required; and
CPR Certification, First Aid Certification, and an annual Tuberculosis test.
During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
EDUCATION and/or EXPERIENCE:
Two (2) years case management experience is required.
Graduation from an accredited four-year college with a degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or a related fieldis preferred .
Associate degreeis required.
