Experience & Requirements
Bachelor's degree in Healthcare related field.
Experience in case management, discharge planning and providing services to a physically and mentally ill population preferred. Prior corrections or re-entry experience is also desirable.
Required to exert up to 50 pounds of force to lift, carry, push, pull, or otherwise move objects including the human body.
Required to kneel, stoop, crouch and/or crawl occasionally.
Frequent walking throughout site; to include walking up and down flights of stairs.
Must have and maintain sufficient strength, agility, and endurance to perform during stressful (physical, mental and emotional) situations encountered on the job.
Occasional running when responding to emergencies; must be able to perform BLS.
A background check, security clearance and drug screening are required for this role.

Responsibilities
Below is a list of your responsibilities as a Re-Entry Coordinator with YesCare:
Development of linkages and referral relationships between the Department of Corrections and local community based organizations, primary care providers, community mental health/developmental disabilities centers, substance abuse programs, and crisis response teams to insure continuity of care after offender release from DOC.
Works collaboratively with the Transitional Housing Unit casework, medical and mental health staff to facilitate the discharge planning process and achieve the goal of a successful transition and reentry to the community.
Develops a multi-systemic needs analysis that documents external linkages and matches the needs with available community supports. Develops, maintains, and updates a database of community healthcare providers for the purpose of facilitating discharge planning and successful reentry to the community.
Ensures the completion of all necessary Release of Information Forms. Documentation of progress notes in the appropriate electronic file such as OPII or other approved DOC or P & P system.
Monitors discharge-planning activities and results. Monitors and recommends strategies to reduce recidivism rates of discharged DOC offenders.
Maintain confidentiality and other standards as set forth by HIPAA, NCCHC, and other state and federal guidelines.
Develops strategies to improve access to healthcare for discharged offenders. Develops CQI studies in collaboration withYesCareCQI Coordinator and site-level Health Service Administrators to determine and increase effectiveness ofYesCare's reentry and recidivism reduction processes.
Participates in any committees/meetings as deemed necessary by DOC orYesCare.
